The Homecare/Assisted Living Provider of the Year will be judged on the applicant’s (2000 words max) entry and supporting evidence

Please include as much supporting documentation (visuals, photos, charts, etc) as possible to strengthen your application.

In no more than 250 words, summarise why your organisation deserves to win this award
Applicants are also encouraged to highlight financial/budget management, staff training and new innovations that have been introduced.
The application should include information on areas served, staff reporting, staff vetting, display excellence in drug/medication management and staff training.
It is important that each applicant provides information on regulatory compliance systems put in place to ensure and display cooperation with current standards and regulations.
Judges would like to see the positive impacts your home care services have had on the community and feedback from patients/clients as to why they continue to use your services.
Applicants must show their plans going forward to further enhance their services for growing community demands and upcoming generations.
